Who is eligible for Home Health Care?

A patient must under the care of a doctor, and the patient is getting services under a plan of care established and reviewed regularly by a doctor.

A doctor needs to certify that the patient has, one or more of these:

  • Intermittent skilled nursing care (other than drawing blood)
  • Physical therapy

The patient must homebound, and a doctor certifies that patient is homebound. To be homebound means:

  • The patient has trouble leaving your home without help (example using a cane, wheelchair, walker, or crutches; special transportation; or help from another person) because of an illness or injury, or leaving your home is not recommended because of your condition.
  • The patient normally unable to leave their home, but if able to it requires major effort.

The patient may leave their home for medical treatment or short, infrequent absences for non-medical reasons, like an occasional trip to the barber, a walk around the block or a drive, or attendance at a family reunion, funeral, graduation.  The patient can still get home health care if they attend an adult day care or religious services.

As part of the certification of eligibility, a doctor, or certain health care professionals who work with a doctor (like a nurse practitioner), must document that they’ve had a face-to-face encounter with the patient (like an appointment with your primary care doctor) within required timeframes and that the encounter was related to the reason the patient need home health care.

Private Pay

Services are also available on a private pay basis for those who choose additional care not covered by Medicare. We offer a series of flexible payment options to meet your desired needs. Contact our office for further information.
